0-17 Years $30/Month*
*Provided an adult family member is also enrolled.
18-59 Years $80/Month
*Provided an adult family member is also enrolled.
60+ Years $110/Month
*Provided an adult family member is also enrolled.
Members will pay a low monthly fee, and receive:
- Free office visits and treatment
- Personalized, truly patient-centered care

More time with and access to Dr. Bergland, including virtual visits, after hours access and in some cases,
house calls - Significant discounts in prices for services such as lab tests, medications, imaging and pathology
Members will NOT receive:
- Bills for office copays
- Surprise after the fact charges that were hidden at the time of service
- Expectations to come in to the office for issues that could safely be handled by phone, email or text message
